Abstract

Bacteria and viral diseases are very common nowadays. Introduction

Hand, foot and mouth is a contagious disease and is more common in children under the age of five. It occurs generally in summers. It can easily spread through direct contact with saliva, mucus, and faeces of an infected person. This disease was first reported by Robinsons and Rhodes in 1958. In initial stages this disease is mild but goes away on its own after some time. This disease is highly communicable and spreads very fast from one person to another. It is mainly caused by Coxsackievirus. This disease can spread from unwashed hands or contaminated faeces. The common symptoms shown by this disease are high-grade fever, sores, skin rashes and blisters. This disease can be preventable by some methods such as regularly washing hands, and avoiding close contact etc. Let’s discuss in detail.

Causes

Hand, foot and mouth diseases spread easily from one person to another by

  • Direct contact
  • Due to cough and sneezing
  • Contact with contaminated objects
  • Contact with secretions such as saliva or nasal mucus
  • Faeces
  • Fluid from blisters

Basically this disease is caused by viruses. These viruses belong to the enterovirus family. Most common viruses that cause this disease are as follows

  • Coxsackievirus A16
  • Coxsackievirus A6
  • Enterovirus 71

Symptoms

Fever and flu like symptoms

These symptoms appear after 3 to 5 days after transmission. Following are the symptoms shown by this disease are

  • High grade fever
  • Loss of appetite
  • Sore throat
  • Tiredness

Mouth sores

These sores are very painful. Firstly they are very small and red spots but after some time that blister comes painful. Due to formation of these sores following signs may appear

  • Difficulty in eating or drinking
  • Patient wanting only cold fluids
  • Excessive saliva

Skin rash

In this skin rashes may appear on the hands, feet and other parts of the body.

Ayurvedic overview

ABSTRACT

Dermatitis is an inflammation or irritation of the skin caused by various external and internal factors. It is caused by an active immune system, infections, allergies, irritating substances, and more. In general, dermatitis can cause symptoms like rashes, blisters, itching, scaling, swelling, pain, burning sensation, etc. It is quite disturbing and discomforting. Dermatitis is a non-contagious skin condition. There are many types of dermatitis like atopic dermatitis, seborrheic dermatitis, contact dermatitis, diaper dermatitis, etc. Phytophotodermatitis is one of them. It is a type of contact dermatitis. We shall be discussing here the detailed description of phytophotodermatitis, its causes, and its management in contemporary as well as traditional systems of medicine.

Phytophotodermatitis

Introduction

It is very much clear from the name itself phyto stands for plant, photo stands for ultraviolet rays or sunlight and dermatitis stands for inflammation of the  skin. Phytophotodermatitis is a non immunogenic inflammation of the skin that occurs due to contact with certain plants which contain photosensitising substances called furocoumarins (psoralens) which get activated following the exposure of sunlight (especially ultraviolet rays ). Several hours after exposure, burning erythema occurs, followed by edema and formation of vesicles (bullae). Wet skin, heat, sweating enhance the phototoxic effect. This results in hyperpigmentation which may persist for several weeks to months. According to the ayurveda dosha involved here is bhrajaka pitta (type of pitta that lives in the skin) which causes rakta dhatu dushti.

Causes and Aggravating Factors

  • Exposure to phytotoxic plants and their associated products
  • Heat
  • Sunny weather
  • Using certain perfumes or oils that contain certain chemicals.

NOTE:- Most phototoxic plants belong to the family mbelliferae, Rutaceae, compositae, and moraceae families. Fennel, fig, wild carrot, lime, etc are some of the phytotoxic plants.

Signs and Symptoms

Usually the symptoms of phytophotodermatitis start within 24 hours and peak around 48-72 hours. Some of the symptoms are as follows:

  • Itching
  • Redness or discolouration
  • Swelling
  • Blister over large area
  • Burning sensation
  • Pain
  • Tenderness
  • Crusted patches of skin when blisters busted
  • Hyperpigmentation

Diagnosis

Diagnosis of phytophotodermatitis is based on the history of the person by  asking about exposure to plants or sunlight or any chemical substance  and carrying out physical examination. If still not clear further tests may be required like patch test or skin biopsy.

Differential Diagnosis

Phytophotodermatitis is often misdiagnosed for certain conditions given below:

  • Cellulitis
  • Burns
  • Fungal skin infection
  • Photosensitivity due to certain drug
  • Urticaria (hives)

Preventive Measures

  • Avoid the contact with plant that may causing skin reaction after identifying
  • Washing the skin especially hands after the contact
  • Using sunscreen lotion with high SPF
  • Covering up the skin while going out

Management

  • Cold compression or ice pack over the affected area can provide some relief
  • Corticosteroids (oral)
  • Anti inflammatory
