Ayurvedic Aspect And Management Of Bitemporal Hemianopia

Abstract

The eye is a complicated and delicate structure that enables a human being to see the world. The human eye is a precise system that is composed of various components that must be optimally maintained so that a clear image is seen. The eye is filled with aqueous and vitreous humor which supply nutrients to the eye and provide its shape. As an external organ, the eyes are prone to get harm from light or injury. The neurology of vision is more complicated in how the sensory impulses are carried by nerve fibers. This is done through a complicated visual pathway. Any discontinuity in the visual pathway can result in blind and other vision-related problems. In the text below, we will discuss Bitemporal Hemianopia. Bitemporal Hemianopia

Introduction

Hemianopia denotes the loss of half of the field vision. It is sometimes also called hemianopsia. Bitemporal hemianopia means a person cannot see the outer half from both eyes. It is usually due to lesions or compression of the optic chiasma. This compression can have a number of causes such as tumors. This usually results in loss of vision in the vertical half of the visual acuity on the lateral sides. Along with this, there can be other associated irritability and symptoms. The condition can be temporary or permanent depending on the cause. Now let’s have a look at the causes, symptoms, investigation, and management of Bitemporal Hemianopia.

Causes

Compression of the optic chiasm affects the visual impulses from both nasal retinas and results in no view of the temporal or peripheral sides. The common causes of this compression are given below.

  • Tumors located at the mid chiasm
  • Pituitary adenomas
  • Craniopharyngiomas
  • Meningiomas
  • Aneurysm of the anterior communicating artery

Symptoms

Ayurvedic Aspect and Management of Bitemporal Hemianopia

Ayurveda is an ancient-age medicinal and life science that is continuously serving mankind. Ayurveda focuses on dietary as well as lifestyle management. Ayurveda has three basic principles/body humor/doshas/energies. When these doshas stay within the normal limits are called the status ‘Health’ whereas vitiation of the same gives rise to a disease.

In various Ayurveda textbooks, akshi rogas (eye disorders) are mentioned. The anatomy and physiology of the same have also been mentioned in the books. Ayurvedic acharyas were so experienced in eye disorders that they have mentioned a number of eye disorders and have well-defined the anatomy and physiology of the eye.

Coming to Bitemporal Hemianopia, there is no direct correlation between this condition with any of the eye diseases mentioned in Ayurveda. But as per the causes and symptoms, we can well establish the correlation of the same with tridoshaja dushti. According to the compression theory behind the occurrence of the condition, it can be correlated with the kapha dosha aggravation that can result in the formation of a tumor that compresses the optic chiasm. When the nerve is compressed it results in nerve irritation that reflects the involvement of the vata dosha and the symptoms such as distorted, double, and loss of visual acuity reflects the involvement of the kapha dosha. Hence, the involvement of all three doshas has been considered.

Another concept that appears to be of good relation with bitemporal hemianopia is Tritya Patalgaya Timira which is mentioned under drishtigata rogas and affects the visual pathway. The symptoms are generally the same but the patient witnesses bilateral horizontal half-vision loss.

The management can be based on the timer chikitsa mentioned in the classical Ayurvedic texts. Chikitsa sutra of timer includes snehapana, akshi tarpan, nasya, and chakshushya basti. (Based on a case report)

Conclusion

Bitemporal Hemianopia is the loss of half of the vision field vertically on the lateral sides of both eyes. This can result due to various causes such as an infection or compression of the optic chiasm. This results in a number of symptoms and the patient are generally irritated as one cannot see properly other symptoms such as double vision and distorted images hamper the concentration of the patient if one tries. This can be temporary and permanent. There is no perfect management available in the contemporary medicinal system but in Ayurveda, the condition can be well managed with the proper application of panchkarma and akshi tarpana. Some systemic medicine also helps a lot to treat the causative factor and corrects the condition of bitemporal hemianopia to the maximum.

Introduction

Abdominal Cocoon Syndrome is a rare cause of intestinal obstruction in which small intestinal encapsulation is done by a fibrous-collagenous membrane. The encapsulation can be partial or total. This is given synonyms of sclerosing encapsulating peritonitis and peritonitis chronic fibrosis incapsulate. There is no cause known for the occurrence of the condition. Usually, patients report to the OPD with symptoms of partial bowel obstruction. In India and Pakistan, the endemic areas, the condition mimics intestinal tuberculosis. This is most common in adolescent young girls. In Ayurveda, this thing can be related to the buildup of ama (endotoxins) and aggravation of the kapha dosha that hampers the activity of the vata dosha. Ayurvedic aspect and management of Abdominal Cocoon Syndrome 

Ayurveda is a life science based on three principles of doshas or three energies namely vata, pitta, and kapha. These three when in normal quantity define health whereas their altered condition is regarded as a state of disease. Ayurveda has been serving the mankind for ages. In Ayurveda, diseases are managed with certain procedures, therapies, and medications.

Coming to abdominal cocoon syndrome, this condition has no direct correlation with any of the disorders mentioned in Ayurvedic texts. Conclusion

Abdominal Cocoon Syndrome is a rare cause of intestinal obstruction. This is caused by an abnormal sac of fibrous collagenous membrane. The cause of the condition is unknown.
