Ayurvedic Treatment Of Scleroderma (Systemic Sclerosis)

ABSTRACT

The human body is made up of multiple organs and every organ has its specific work and function. For proper functioning of the body, energy is needed. When there is exposure to foreign particles, our body’s natural immune system gets activated and helps against the foreign particle exposure. This is called immunity. When for any reason, our body’s immune system mistakenly attacks its own healthy tissues, then this condition is known as Autoimmunity. There are several diseases in which our body’s immune system attacks its own tissues and generates diseases which are known as autoimmune diseases. Here, we are going to discuss such a disease which occurs as a result of Autoimmunity. Scleroderma/Systemic Sclerosis is a type of disease in which the body’s natural immunity is not functioning properly and attacks its own healthy tissues. Scleroderma is a disease with unknown pathogenesis. It is a rare connective tissue disorder. Let’s start a detailed study about Systemic Sclerosis/ Scleroderma.

Scleroderma/Systemic Sclerosis

INTRODUCTION

Scleroderma is a group of autoimmune disorders which is very rare. In this disease, the skin becomes hardened and tightened. Scleroderma is made up of two words that are Sclero (Hard) and Derma (Skin). In this disease thickening and inflammation occurs in the skin and other body areas. It is a perfect example of an autoimmune disorder in which the immune system can not differentiate between healthy tissues and foreign particles. Due to this autoimmune effect, the immune system triggers the body to produce large amounts of collagen protein. Collagen is a protein which in normal amounts helps in supporting the health of connective tissues. When the collagen forms in a large amount it makes skin tissues thicker than normal. TYPES OF SCLERODERMA

Scleroderma can be divided into two types according to its location of attack which are as follows

  1. Localised Scleroderma : It is known as Localised Scleroderma when it affects the skin tissues. Localised means present in one area. Usually, it does not spread to other areas of the body which means only one part of the body is affected.
  2. Systemic Scleroderma : It is known as Systemic Scleroderma which also affects the internal organs of the body like the digestive system or respiratory system etc. It is more serious as compared to Localised Scleroderma as it affects internal organs. Systemic Scleroderma may be diffuse or limited. In Diffuse type of systemic scleroderma, the thickening of the skin is spread to large areas. As it spreads to wide areas it can affect multiple organs at the same time. In limited types of systemic scleroderma, it spreads to a limited area with the symptoms of extra calcium deposition, esophageal dysfunction, skin tightening and numbness of the skin.

CAUSES OF SCLERODERMA

The cause of Scleroderma is not known exactly. But there may be some possible factors which can lead to the disease of Scleroderma

  • Immune system : It is an immunity-related disorder in which the immune system accidentally attacks healthy tissues of the body.
  • Genetics : It can pass from genes from parents to their children.
  • Environmental factors like any exposure to chemicals, viruses or any other microorganisms etc.

    According to the above Ayurveda text, causes of Scleroderma are as follows

  • Salty, sour, pungent, acrid, unctuous, hot, undigested food.
  • Dry food and fish intake
  • Eating before digestion of previous food
  • Sleeping in daytime
  • Excessive sleep
  • Excess anger
  • Lack of exercise
  • Consumption of sweet taste.

SYMPTOMS OF SCLERODERMA

According to the above text, the signs and symptoms are

  • Excessive or unable to sweat.
  • Black discolouration of the body
  • Insensitivity to touch

DIAGNOSIS OF SCLERODERMA

Diagnosis of Scleroderma requires a thorough examination and physical and medical history. Following are some of the diagnostic tests that can be performed to diagnose Scleroderma

  • ANA (Antinuclear Antibody Assay) test to screen Scleroderma and other connective tissue disorders.
  • Nailfold Capillary test which helps in testing whether nail capillaries are larger or normal. This test is important in the examination of any abnormality in nail vessels
  • Skin test to measure the skin thickness
  • Pulmonary function test in case the respiratory system is involved
  • Upper GI testing when the digestive system is involved

AYURVEDIC OVERVIEW OF SCLERODERMA

Scleroderma is a rare autoimmune disorder in which the skin of the body becomes thicker than normal. Usually, it affects the upper skin of the body but it may also affect the inner organs of the body. It is an autoimmune condition in which the body’s immune system mistakenly attacks the healthier cells of the body instead of protecting the body. The immune system in this condition is not able to differentiate between the body’s tissue and foreign invaders. In Ayurveda, Scleroderma can be correlated with Uttan Vata Rakta associated with Twakgata Vata Vyadhi. According to Ayurveda, it is a Yapya Vyadhi (incurable but can be manageable). Vata (dominant) and Rakta (Associated) are the Doshas involved in these conditions. The Dushyas involved are Rasa, Rakta and Mamsa.Vitiation of these Doshas and dushyas can lead to the condition of Uttan Vata Rakta.

According to the above text, the diet in Scleroderma is as follows

  • Old grains, Raktashali rice

  • Cow’s milk, goat milk or buffalo milk

  • Bloodletting by the horn, leech, needle, gourd or scratching after assisting the dosha and strength of the patient.
  • In patients having complaints of pain, burning sensation, colic pain and pricking pain leech therapy can be done.
  • Bloodletting by Horn is done in the patients having complaints of numbness, itching and tingling sensation.
  • If pain radiates from one part to another part then scratching can be done.
  • In the case of Excessive dryness in the body parts, bloodletting should be avoided.

Samshodhana chikitsa (purification therapy) can be used as follows

  • Virechan (therapeutic purgation) can be given after Sneh (medicated ghee) administration
  • Alternative administration of Anuvasana and Niruha Basti (Therapeutic decoction enema)

Growing older is linked to significant alterations in body composition, with the main cause being a reduction in skeletal muscle mass. Sarcopenia is the term used to describe this age-related loss of skeletal muscle. The age-related decline in muscle strength is directly connected to the age-related reduction in muscle. The main factor influencing the age- and sex-related variations in strength appears to be muscle mass rather than function. The location (upper vs. lower extremities) and function (extension vs. flexion) of the muscles have no bearing on this connection. One of the main reasons why older people have higher rates of impairment is because they have less muscle mass. Muscle strength is an essential component of walking ability, especially in the elderly due to their advancing age and extremely low activity levels. It’s possible that the elderly living in institutions have a higher risk of falls because of their weakened muscles. Over the course of adulthood, daily energy expenditure gradually decreases. Fat-free mass is the primary factor that determines energy expenditure in sedentary individuals. It decreases by around 15% during the third and eighth decade of life, which leads to a decreased basal metabolic rate in older adults. Research suggests that sarcopenia can be avoided and muscle mass preservation can help stop the decline in metabolic rate. Skeletal muscle plays a crucial part in energy metabolism, and as we age, it may also be linked to age-related changes such decreased bone density, insulin sensitivity, and aerobic endurance.

Age-related loss of muscle mass and strength is a common skeletal muscle condition known as sarcopenia. It is increasingly acknowledged as a primary contributor to morbidity and disability in the aged. Sarcopenia may start in a person’s fourth decade of life. Skeletal muscle mass and strength appear to decrease linearly with age, with up to 50% of mass lost by the eighth decade of life, according to the evidence. A musculoskeletal condition known as sarcopenia is characterised by a progressive decrease of muscle mass and strength, especially in older people. Reduced levels of muscle strength, muscle quantity or quality, and physical performance are all included in the diagnosis of sarcopenia. Such musculoskeletal deterioration has been shown to have predictive value for adverse postoperative outcomes, higher rates of complications, death, and morbidity following major surgical procedures, and the inability to carry out daily tasks. Furthermore, there is a clear correlation between sarcopenia and a higher risk of fracture and a higher frequency of falls. Additionally, in people older than 90, loss of independence is associated with sarcopenia’s criteria of decreasing muscle mass or function. CAUSES

The aging process is the most frequent cause of sarcopenia. You start to gradually lose strength and muscular mass in your 30s and 40s. The years 65 to 80 are when this process accelerates. Every ten years, you may lose up to 8% of your muscle mass, however rates can vary. Although everyone gradually loses muscular mass, sarcopenic individuals lose it more quickly. Although aging is often the primary reason, other potential risk factors for sarcopenia have been found by researchers. These could include:

In medicine, the gradual loss of muscle mass, strength, and function is called sarcopenia. Sarcopenia is the term for the progressive loss of muscle mass and strength associated with aging. The main sign of the illness is muscle weakness. Sarcopenia, or the atrophy of the muscles, is primarily caused by ageing. Scientists have identified two risk factors for the condition: consuming unhealthy foods and not exercising. Sarcopenia and mamsa dhatu kshaya (ageing) are connected, according to Ayurveda. Dhatu kshaya arises when the vata dosha becomes overly vitiated with age. Certain herbal therapies are beneficial and useful for sarcopenia according to Planet Ayurveda.
