Abstract

Mallet finger is a disease of the tendon of the finger. Tendons are the tissues or like a rope made up of collagen fibers that connect the muscles to the bones. Long tendons attach the muscles through the wrist and attach to the finger bones and thumb. Tendons help the joints for extension and flexion. The extensor tendon helps in straightening the fingers and the flexor tendons help in bending the fingers. Tendons link the muscle to bone and have great tensile strength so that joints can move freely. But in tendon injury, there is a difficulty in moving joints of the body. So in this article we are going to discuss one of the tendon diseases that is Mallet finger and its Ayurvedic treatment.

Introduction

Mallet finger is the injury to the small tendon of the finger and thumb which is helpful in the extension of the finger. This is the term applied to the extensor avulsion fracture of the finger or also we can say distal extensor tendon rupture. There is another term for this disease that is baseball finger. This is a deformity of the fingertip which resembles a mallet or hammer. When this injury occurs, a person finds it difficult to extend the finger tip without pushing it and also there is generally pain and bruising at the back side of the finger joint. This is a result of over bending of the finger tip by a sudden attack on the finger by a ball or something else.

Etiology/Causes

This typically occurs when a ball hits hard on an outstretched finger and jams it. This results in the tear or rupture of the tendon of that finger. A direct hit on the extended finger during a game like baseball, football, cricket, volleyball, basketball, etc can cause the injury to the tendon of the tip of the finger.

Clinical Features/Signs And Symptoms

The person who has this mallet finger they have following symptoms:

  • Pain
  • Swelling
  • Bruising
  • Tenderness
  • Redness
  • Inability to straighten the fingertip

Diagnosis

The diagnosis is generally made on clinical examination and supported by X-ray. This injury can be accompanied by ecchymosis and swelling.

Abstract

Adenomatous polyps are basically defined as the neoplastic benign epithelial polyps of the colon. By convention they are termed as adenomatous or sometimes tubo glandular adenoma. These lesions occur sporadically in the general population and also in certain well defined hereditary syndromes such as heredofamilial multiple polyposis.in one large series the average age of patients having sporadic adenomatous polyps was 60 years with a male preponderance in the ratio 2:1. Their frequency generally increases after 30 years of age. In this article we will typically discuss adenoma which is one of the types of adenomatous polyps including its Ayurvedic management.

Introduction

Adenomas are divided into three subtypes usually based on their histologic criteria. The types are tubular, tubulovillous and villous. The tubular adenomas are encountered most frequently around 80%-85%, Tubulovillous adenomas are encountered less frequently like 8%-10% and villous adenomas are least encountered that is below 5%. The Average age of patients having such a lesion is in the range of 60-65 years and they are uncommon under the age of 50 years. Villous adenoma, while rarely stalked, are almost always sessile lesions, attached directly to the colonic mucosa by a broad base. Most often they occur singly in the rectosigmoid (80%-90%) but rarely multiple villous tumors are present.

Causes of Villous Adenoma

The causes associated with onset of villous adenoma are not commonly defined but following are the factors which can be considered:

  • Genetic factors
  • Lifestyle and diet
  • Acromegaly
  • Inflammatory bowel disease
  • Ulcerative colitis

Symptoms And Signs of Villous Adenoma

Villous adenomas are symptomatic much more frequently than are adenomatous polyps. The symptoms generally observed are as following:

  • Most commonly they cause rectal bleeding
  • In addition those containing goblet cells may hypersecrete copious amounts of mucoid material that is sufficiently rich in protein to produce hypoproteinemia. Particularly hypoalbuminemia.
  • The mucus secreting contains large amounts of potassium causing hypokalemia.
  • Persistent diarrhoea is rarely present. But if present it can cause significant loss of fluids and electrolytes.
  • Flatulence or bloating might also be present.
  • Constipation may be present.

Appearance of Villous Adenoma

The villous adenoma are velvety, cauliflower, fungating appearance and range from 1cm to 10 cm in diameter. The pale grey lesions are elevated about 1-3 cm above the surrounding mucosa  and often show focal areas of hemorrhage or surface ulceration. Histologically they are composed of finger-like or sometimes branching papillae which are responsible for their soft velvety appearance.

Diagnosing Villous Adenoma

The main investigation for treatment are as follows:

  • Flexible sigmoidoscopy
  • Double contrast barium enema
  • Colonoscopy
  • Polypectomy during an endoscopic procedure

Treatment of Villous Adenoma

  • Polyps at the rectum can be removed during a sigmoidoscopy
  • At colonoscopy the polyp which is usually pedunculated is removed with a snare and recovered for historical examination to assess the completeness of removal and the presence of premalignancy or invasive malignancy.
  • Surgical polypectomy is needed or is necessary if there has been incomplete removal of a malignant polyp. Surgical polypectomy is generally done in sessile polyps, particularly in villous adenoma.
