Abstract

Normally in our body cells grow and divide and later on die and get replaced but in cancer these cells grow abnormally and spread to whole tissues of the cell wall and grow as a tumor (a growth or lump). Tumors can be benign or malignant. Benign tumors are not cancerous but malignant growth are cancerous. They can be solid tumors or the cancer of the blood called leukemias. Benign tumors do not spread to other parts and remain localized to one part of the organ and after removal it doesn’t grow back. But sometimes benign tumors are dangerous as in the brain. So in this article, we are going to discuss an anticancer drug named Dostarlimab and see what are the alternatives of Dostarlimab in ayurveda!

Introduction

Dostarlimab is a drug which is used to treat cancer, is a IgG4 monoclonal antibody which is targeted against Pd-1. The cancer which is invaded into the tissues grows uncontrollably and spreads to the whole parts of the body. In normal body structure the cells grow only when they receive the signal. Cancer is a genetic disease, and is inherited from the parents and many other environmental factors affect the growth of cells. The genetic changes which are affecting are: proto – oncogenes (they are involving normal cell growth and that cells start working more actively and causes cancer causing genes in the body), tumor suppressor genes (they are controlling the normal growth) Dna repair genes (they are helping in fixing the damaged Dna where they causes mutations and this mutation leads to duplication and at end lead to cancer growth).

Spread Of Cancer

Cancer spread from site of origin to another place is called metastatic cancer, and this process is called metastasis. We can examine this metastatic cancer under a microscope which appears the same as the original cancer.

Types Of Cancer

There are many types of cancer which start from the name of the organ such as lung cancer starts from lung, brain cancer starts from brain, and many cancers formed from cells that are epithelial cells, squamous cells.

1. CARCINOMA – Formed by epithelial cells and the most common type of cancer. Are present in different cells – such as adenocarcinomas that produce fluids called glandular tissues.
2. SARCOMA – Forms in the bone (osteosarcoma), in the lymph vessels, and in the fibrous tissues (tendons and ligaments).
3. LEUKEMIA – Cancer which begins in the blood and causes leukemia. Forms large numbers of white blood cells are built in the bone marrow.
4. MELANOMA – It is the cancer of the pigment known as melanin. Can also be seen in the eyes.
5. TUMORS –
   1. Germ cell tumor can be found at any part of the body but generally arises from the cells giving rise to sperms and eggs.
   2. Carcinoid tumors are a type of neuroendocrine tumor. Are mostly found in the gastrointestinal system and spread to the liver and many other parts of the body.

Causes Of Cancer

• Family History – Can be due to some genetic mutations, or can be exposure to certain medicines.
• Lifestyle Changes – Main factor that causes cancer is sedentary lifestyle and high fat diet, smoking.
• High Dosage Of Chemicals – Infants are more prone to cancer due to exposure to insecticides, fertilizers, and pesticides.

Efficacy Of Dostarlimab In Treating Cancer

Dostarlimab is used in patients who are suffering from recurrent endometrial cancer. It is a monoclonal antibody which binds to the Pd-1 receptor and it blocks the action of proteins in cancer cells. Pd -1 is a bound to protein Pd-L1 which kills the cells, and Pd-1 manages the immune system of the body. When Pd-1 acts it facilitates the antitumor activity and treats many cancers. Dostarlimab is given for 30 minutes via iv infusion every 3-6 weeks. Dostarlimab is a life threatening medication which can cause many symptoms in patients so it should be given strict guidance of nurses and doctors.

Indications

Given in patients who are not treated with chemotherapy, endocrinopathies, hepatitis with tumor involvement of the liver. And with patients who have been treated but there is recurrence of cancer causing cells. This drug slows down the growth of tumors or slows down its speed. Dostarlimab is blocking certain channels of the protein pathway.

Special Precaution Before Taking Dostarlimab

• Patient must inform the doctor about his / her allergies.
• Patient must tell him about his medicines he / she is taking such as – multivitamins, nutritional supplements, so that the doctor may take more care of the patients.
• If patient is suffering from any disease such as ulcerative colitis – which causes sores in the lining of the colon), Guillain barre syndrome (which is caused due to nerve damage), any liver disease, thyroid disease hypothyroidism, hyperthyroidism, myasthenia gravis (rapid weakness of the muscles).
• If a patient is breastfeeding then this drug is not advised.

Side Effects Of Dostarlimab

• He / She may feel weakness in joints
• Increased sweating
• Increase in appetite
• He / She may listen to voices that don’t exist.
• He / She may feel fever, nausea, vomitings, sometimes this medication causes seizures, eye pain, stomach pain.

Ayurveda Aspect on Dostarlimab

Cancer is a disease which is common in many countries worldwide , and the main cause of it is an imbalanced diet or more use of dirturbed food habits of every person. According to acharya, sushrut (father of surgery) said that cancer is caused due to accumulation of aggravated doshas in any part of the body – as in excess growth which is caused in mammals called mamsa arbuda. According to doshas there are many types of cancer –

1. Vataj arbuda
2. Pittaja arbuda
3. Kaphaja arbuda
4. Rakta arbuda – This condition gives rise to anemia because these doshas take their place in vessels (sira).
5. Medo arbud – This is the metastatic growth.

Drugs used in Treatment of Cancer

1. Devadaru (Cedrus deodara): It is known as himalayan cedar which is very useful in neurological disorders. There are two varieties of devadaru – devadaru (cedrus deodara) and sarala devadaru( pinus longifolia). Devadaru is tikta (bitter) katu (pungent) kashaya (astringent) in taste. It has rookshan (dry) guna. Its vipaka is katu and has ushana veerya. It balances kapha and vata dosha. The parts used are bark, leaves, resins. It is very useful in indigestion and altered metabolism. Treated in obesity.
2. Aragwadha (Cassia fistula): It is mainly used in constipation and in many skin diseases. It has mrudu (soft), guru (heavy), and snigdha (oily) gunas. Its vipaka is converted after digestion. It has sheeta veerya. Balances kapha and pitta dosha. It has cardioprotective properties and blood purifiers too.
3. Langali (Gloriosa superba): Used in treatment of wound healing, during labor if one feels difficulty, or worm infestation. Langali has katu (pungent) tikta (bitter) rasa. Has ushana veerya, its main action is to abortifacient, and is kapha and vata shamaka.
4. Kshara – It has alkali property and is used to relieve the tridosha. Kshara is pungent and has ushna veerya , it heals the wound and has stambana property.

6. Chandrprabha Vati

Chandraprabha vati is one of herbal formulation of planet ayurveda which consists of asphaltum (shilajit), commiphora mukul (guggul), sugar, cinnamomum camphora (karpoor), acorus calamus (vacha), cyperus rotundus (mustak), piper chaba (chavya), embelia ribes (vidanga), tinospora cordifolia (guduchi), piper longum (pippali). This formulation is Used in thyroid disease, kidney pancreas and it brings glow to face and body and it provides strength and increases immunity. It is Very useful in ashmari, vibandha (constipation), mutraghata (urinary obstruction) pandu (anemia). Chandraprabha vati is very useful in reducing the sugar content in urine, and it decreases pain and cramps and plays an important role in menstruation and provides strength to muscles of the uterus.
