Abstract

Bilirubin is a yellowish pigment produced by the breakdown of red blood cells. It is a waste product that is produced and excreted from the body simultaneously. If the quantity of bilirubin increases in the body, it shows various symptoms and can cause prolonged adverse effects. Generally, Gilbert’s Syndrome isn’t noticed until you go through a blood test. Gilbert’s Syndrome 

Introduction

Gilbert’s Syndrome is not a serious condition. This liver does not function properly and does not produce normal quantities of bilirubin. The mutated gene affects the ability of the liver to process bilirubin formation properly. Unexplained and recurrent jaundice can be a common symptom of Gilbert’s Syndrome due to the accumulation of bilirubin in the bloodstream. Either it may have no ill effects on your body until you get to know it through blood tests done for other purposes. Fasting for a long time, menstrual cycle, and excess physical work can trigger this condition. Also, prolonged increased bilirubin levels can lead to gallstones and other serious liver diseases.

Ayurvedic Aspect

Excess bilirubin levels in the body cause jaundice-like symptoms and can be correlated with “Kamala” in Ayurveda. Kamala is said to occur due to excess Pitta Dosha in the body that affects Rakt dhatu and Mams dhatu (Blood and muscles) causing Yellow discoloration of skin and urine. Depending on the doshas involvement Kamala is characterized into three types namely-

  • Shakhagatashrit Kamala (Obstructive jaundice)
  • Koshthaashrit Kamala (infective jaundice increased bilirubin levels)
  • Koshth-shakhaashrit Kamala (Haemolytic jaundice)

From the above three types of Kamala, the Koshth-shakhaashrit Kamala is a rare and serious condition that represses all the seven dhatus that cause severe weakness and deformities in the body.

Prolonged untreated Kamala causes increased levels of bilirubin with more complications and Excess Vata dosha Prakop and is correlated with “Halimak” and “Laghrak” in Ayurveda.

Causes

Gilbert’s Syndrome occurs due to an inherited gene that leads to an increase in blood bilirubin levels. Bilirubin levels in the body also increase due to-

  • Infections
  • Illness
  • Dehydration
  • Low intake of a nutritious diet
  • Menstruation in females
  • Overexertion
  • Stress

Symptoms

Common Symptoms of Gilbert’s Syndrome are

Treatment

Since Gilbert’s Syndrome is not a serious problem, the treatment is not found necessary. Still, some medications are used to lower bilirubin levels and are prescribed for jaundice.

The intestine is a well-known tube-like part of the body that extends from the stomach (from the lower end) to the anus (the lower opening of the digestive tract). Tuberculosis may infect any part of the body, mainly the lungs but the incidence of tuberculosis is very common in the intestines too. It is primarily due to the invasion of bacteria in the body and affected the intestines. In this article, we will discuss intestinal tuberculosis  and the ayurvedic aspect of intestinal tuberculosis.

Introduction

Intestinal tuberculosis is a primary infection from ingesting milk infected with bovine tuberculosis or as a secondary spread of pulmonary tuberculosis following the swallowing of infective material coughed up from the lungs. The infection begins in the ileum where the abundant lymphoid tissue traps the organisms. A typical granulomatous reaction may lead to hyperplastic thickening of Peyer’s patches and surrounding bowel wall and create necrotic ulcers on the mucosal surface. These ulcers extend along the encircling lymphatic system of the small bowel. They may penetrate deeply and even  perforate. In both ulcerative and hyperplastic patterns, small miliary tubercles are frequently found in the subserosal layer and sometimes throughout the peritoneal cavity. Intestinal involvement may lead to diarrhea, intestinal cramps, and abdominal pain but in many cases, the symptoms are masked by the infection elsewhere.

Causes

The Following Causes of Intestinal Tuberculosis are

  1. Bacterial invasion (Mycobacterium tuberculosis)
  2. Ingesting infected milk.

Symptoms

The symptoms of Intestinal Tuberculosis are listed below

  1. Non-healing ulcers of the mouth or anus
  2. Weight loss
  3. Low-grade fever
  4. Anorexia
  5. Cough
  6. Difficulty in swallowing
  7. Pain in the abdomen due to gastric and duodenal infection
  8. Infection of the small intestine known as malabsorption
  9. Due to colon infection Diarrhea and pain occurs.

Treatment of Intestinal Tuberculosis

ATT (anti-tuberculous therapy) is the line of treatment for Intestinal tuberculosis. Isoniazid 5 (mg/kg body weight) maximum (mg) 300, Rifampicin 10 (mg/kg body weight) maximum (mg) 600 Pyrazinamide 25 (mg/kg body weight), Ethambutol 15 (mg/kg body weight). This is the basic line of treatment for intestinal tuberculosis.

The ayurvedic point of view of this disease. The ayurvedic point of view of Intestinal Tuberculosis is mainly correlated with Rajayakshma. Rajayakshma is a group of disorders in which all three doshas are aggravated and it also affects Sapta dhatu in our body. Rajayakshma is also known as the king of all diseases. It is caused by the simultaneous vitiation of all the three doshas as a result of suppression of natural urges, performing activities more than one’s strength, and irregular food intake. In rajayakshma, patients become emaciated due to a reduction in the number of tissue elements because of the obstruction to the channels of rasa circulation. Excessive semen ejaculation is also associated with emaciation in this condition.    

Rajayaksma is followed by many diseases and acts as a primary factor in the origin of many disorders. It is also known as Ksaya and shosha. Sosha is known as dried rasa dhatus, which leads to poor strength and activities which are called Ksaya (diminished strength). The Rajyakshma is a Kapha Pradhan tridosha vyadhi associated with vitiation of pachaka pitta and kledaka Kapha. Dusya included Dhatu like rasa and rakta. Srotodushti types include sanga (a coalition of dosha) and Vimarggamana ( impaired pathway or obstructing the channels).
