What is Juvenile Idiopathic Arthritis? – Causes, Types Therapies and Treatment

Abstract

The most frequent kind of arthritis in children under the age of 16 is juvenile idiopathic arthritis, previously known as juvenile rheumatoid arthritis. Joint inflammation, edema, and stiffness are all symptoms of JIA. Some children may only have symptoms for a few months, while others may have them for years.

Introduction

Juvenile idiopathic arthritis may have major consequences, including growth issues, joint damage, and ocular irritation. Controlling pain and inflammation, enhancing function, and avoiding damage are the main goals of treatment. JIA is somewhat more frequent in women than in men. It is more frequent in children under the age of six or teens. JIA is divided into many categories, each with its own set of symptoms. Knowing which kind you have might help you determine how likely you are to grow out of JIA. It is maybe difficult to manage in some individuals, but in the vast majority of instances, medication will suffice. If you develop arthritis as a side effect of another illness, it’s usually treated similarly to JIA.

Juvenile idiopathic arthritis

Causes

The actual causes of juvenile arthritis, however, remain uncertain. JRA, on the other hand, is an autoimmune disease in which the body’s immune system assaults its cells and tissues. It may be triggered by a variety of factors.

  • The three energies or doshas – Vata, Kapha, and Pitta – are responsible for an individual’s health in Ayurveda. Joint problems are caused by aggravated Vata and Kapha doshas. However, with Amavata, the buildup of ama (toxin) in the body produces joint discomfort and inflammation in addition to Vata aggravation.
  • A viral or bacterial infection might be a trigger in the environment.
  • Vata aggravation and ama development are caused by a variety of factors such as lifestyle and poor dietary habits.
  • Mutations in the immune system lead it to function improperly and target its cells.

Natural therapies for the pain of Juvenile Arthritis (include herbs, spices, and vitamins)

  • Turmeric: It is a yellow powder derived from a similar flowering plant that has anti-inflammatory qualities. Turmeric and curcumin supplements may be beneficial in relieving the pain of a kid with Juvenile Arthritis.
  • Green Tea: Green tea has been used medicinally for ages to decrease inflammation in the body.
  • Cinnamon: It includes cinnamaldehyde and cinnamic acid, which are antioxidants that aim to minimize cell destruction caused by free radicals. When combined with other anti-inflammatory foods and spices, it may help offer a cumulative anti-inflammatory effect as part of natural therapy for Juvenile Arthritis.

Juvenile Idiopathic Arthritis Types

Undifferentiated, polyarthritis, juvenile psoriatic arthritis, oligoarthritis, and systemic arthritis are the six forms of JIA.

Exercise & Juvenile Arthritis

Muscle strength and endurance may be reduced in children with Juvenile Arthritis. They may also have a reduction in aerobic and anaerobic capability. During physical exertion, a kid with Juvenile Arthritis may fatigue sooner than their counterparts who do not have arthritis. Studies have demonstrated that exercise training may improve these disorders by increasing aerobic and anaerobic capacity, as well as increasing muscular strength and endurance via resistance training.

Types of Physical Activity

  • Motion-capacity
  • Stretching
  • Strengthening\Cardiovascular

Natural Cure For Juvenile Idiopathic Arthritis

ABSTRACT

Arthritis is an inflammatory joint disease that targets single or multiple joints. It occurs more frequently in females. It can occur at any age. The most common chronic or long lasting type of arthritis that occurs in children is JIA (Juvenile Idiopathic Arthritis). It consists of various conditions involving inflammation, stiffness and pain in joints.

JIA (Juvenile Idiopathic Arthritis) can occur in various forms and patterns. It is very essential to see a doctor if your child develops swelling in joints or experiences pain or stiffness in joints.

Juvenile Idiopathic Arthritis

INTRODUCTION

Idiopathic refers to an unknown pathology, i.e. where the cause is not known. When a child or adolescent under the age of 16 years is affected by Arthritis, it is known as JUVENILE IDIOPATHIC ARTHRITIS(JIA).

Similar to the arthritis that occurs in adults, JIA also is an autoimmune disorder. It occurs when the immune system works in an abnormal manner and leads to various joint related problems such as inflammation, stiffness etc.

CAUSES

JIA (Juvenile Idiopathic Arthritis) is an autoimmune disorder. In healthy conditions, the immune system of the body acts as a defense mechanism. It helps to fight infections and promotes healing of wounds. But in a condition like an autoimmune disorder, the immune system becomes abnormally overactive and starts to attack the healthy cells and tissues.

The exact cause of this abnormal over activity of the immune system is still unknown. Family history of arthritis plays a vital role in making the person more prone to the condition .Various scientists believe that molecules—TNF-alpha, IL-6, and IL-1 play a major role in causing inflammation in joints of children affected by JIA.

SYMPTOMS

JIA (Juvenile Idiopathic Arthritis) may occur in various patterns and hence symptoms differ according to the type of pattern. In all types of JIA, following symptoms can be seen-

  • Joint pain
  • Swelling in joints
  • warmth
  • Stiffness in joints
  • These symptoms become worse in the morning or just after waking up from a nap.
  • The child may or may not complain of pain.

The main patterns of chronic arthritis that commence in children are-

  • Systemic
  • Polyarticular
  • Pauciarticular

Systemic Onset Juvenile Arthritis

Age: Most commonly affected age group-between the ages of 1 and 5 years.

Symptoms

  • Hepatomegaly  i.e. Liver enlargement
  • Splenomegaly i.e. spleen enlargement
  • weight loss
  • growth retardation
  • when the temperature is raised , a macular rash appears and then quickly disappears

Polyarticular Juvenile Chronic Arthritis

Seronegative Seropositive
1. Age can occur at any age usually after 8 years of age.
2.Effect on Joints Four or more large joints usually affected Progression of erosive changes in more than half of the joints.

Pauciarticular Juvenile Chronic arthritis-

In this type of JIA , usually four or fewer joints are involved.
In older boys, the most frequently involved joints are – Hips, Knees or Ankles.

DIAGNOSIS

Various clinical examinations and lab investigations are performed to confirm the condition which are as follows:

  • Radiography- X-Rays reports help in visualizing the condition of joints.
  • Serological tests
  • Blood tests such as hemoglobin levels, erythrocyte sedimentation rates (esr)- Raised ESR levels may be an indication of infection.
  • CT scanning
  • Ultrasound

TREATMENT

The management of juvenile arthritis is similar to that of adult rheumatoid arthritis.
In acute phases, i.e. short term phase

  • Bed rest may be essential
  • Prone lying
  • Appropriate physiotherapy

In severe cases- Corticosteroids are used. In persistently active disease-Immunosuppressants are given.

AYURVEDIC ASPECT

In Ayurveda, Juvenile Idiopathic Arthritis can be correlated with Vata vyadhi. According to Ayurvedic principles, 3 elements: Vata,Pitta, Kapha are the fundamentals of the health of the body. The equilibrium between these three is responsible for the healthy state of the body while any hindrance to any of the three Vata,Pitta, Kapha will lead to the diseased state of the body.

When Vayu is circulating in the body without any hindrance and in its natural state and form, the person becomes capable of living a life of more than a hundred years. Hence, appropriately describing the major role that Vata plays in maintaining the healthy state of the body. It is very essential to maintain Vata in its natural state which  is only possible by avoiding and restricting the consumption of factors that will lead to its disturbance. These factors are very well mentioned in ch.chi.28/15-17 for example:

  • Ruksha ahara
  • sheeta ahara
  • Alp ahar i.e. less quantity
  • adhik vyayam i.e. excessive exercise
  • chinta i.e. stress
  • diva swapna-sleeping in day
  • upvasa i.e. fasting etc
  • krodha i.e. anger etc

By avoiding the above said factors, Vata vyadhi can be prevented.
