Abstract

Glanzmann’s thrombasthenia (GT) is an inherited blood clotting disorder which is characterized  by impaired function of specialized cells called as platelets that are essential for blood clotting (coagulation). The symptoms generally include abnormal bleeding and if left untreated for a long time can prove fatal. GT can be managed in a safe and effective manner with the help of Ayurvedic mode of treatment. Introduction

Glanzmann’s thrombasthenia is a bleeding disorder which is characterized by spontaneous bleeding starting from birth. In Glanzmann’s Thrombasthenia a person tends to bleed easily, may also present with easy skin bruising, can have frequent nosebleeds (epistaxis) and bleeding gums. Such patients can present with purple or red spots on skin which are usually due to bleeding underneath skin or swelling caused by bleeding within tissues. People with this disorder tend to bleed a lot following injury or surgery. Females with Glanzmann’s Thrombasthenia have excessive bleeding during menses and bleed more than usual after child birth. This excessive bleeding can result in extreme blood loss and can be fatal.

Causes of Glanzmann’s Thrombasthenia

This condition is a result of gene mutation. The Mutation in ITGA2B or ITGB3  results in  this condition. This gene is responsible for making two parts (subunits) of receptor protein integrin alphallb/beta3. This protein is abundant on the surface of platelets. Platelets are the small cells which circulate in blood and are an important component for clot formation. During coagulation integrin helps the platelet to combine together. The clot formation protects the body after injury by sealing off the damaged blood vessels and hence bleeding stops.

The gene mutation results in the deficiency of integrin alphallb/beta3. Due to its deficiency the platelets cannot clump together thus resulting in prolonged bleeding.

In some cases of Glanzmann’s Thrombasthenia no mutation is seen in integrin alphallb/beta3. In such cases the cause for the condition is not known.

Types of Glanzmann’s Thrombasthenia

Type 1:  In this type people have less than 5 percent of integrin alphallb/beta3 levels. This is the most common type.

Type 2: In this type people have 5-20 percent of integrin alphallb/beta3 levels.

Variant type: In this type the body produces an adequate amount of integrin alphallb/beta3 but it is nonfunctional.

Symptoms of Glanzmann’s Thrombasthenia

The symptoms in this case include

  • Tendency to bleed starts right after birth or short time.
  • Purple or red spots on skin
  • Bleeding gums
  • Bleeding nose (epistaxis)
  • Excessive bleeding during surgery or after surgery.
  • Intermittent gastrointestinal bleeding resulting in melena
  • In women excessive menstrual bleeding
  • Excessive bleeding during childbirth and after childbirth
  • Intermittent uterine bleeding
  • Blood in urine
  • In severe conditions hemorrhage and this can be fatal.

Treatment of Glanzmann’s Thrombasthenia

In modern science there is no specific treatment for this condition. The treatment   includes preventive measures like protecting oneself from injuries in case to avoid blood loss, some antifibrinolytics drugs to stop excessive bleeding, blood transfusion, if required.

This vati uses standardized stem extract from Tinospora cordifolia plant. 3. Kumar Kalyan ras

Hidradenitis suppurativa is a chronic skin condition characterized by recurrent boil or lump beneath the skin. It causes abscess and scarring on the skin. These usually appear where skin rubs together or near the hair follicles having sweat glands. Groin region, breast, buttocks, and armpits are mainly affected. Women are more prone to this disease. Hidradenitis suppurativa usually starts after puberty and may become worse over time. It can pose serious effects on day to day life and emotional well being.

No, it is not contagious.

What are the sign & symptoms of Hidradenitis suppurativa?

  • It can affect one or multiple areas of the body at a time and symptoms may vary from mild to severe.
  • Blackheads in small pitted areas of skin.
  • Lumps or boils appearing on various areas of the body are very painful.
  • A boil may be pea-sized and firm which with time either disappear or rupture and pus ooze out of it for few hours or maybe days. This cycle continues either on the same area or somewhere else on the body.
  • The hard and painful lumps which appear may develop and enlarge under the skin for years.
  • Oozing and leaking boils or lumps heals very slowly and can lead to scarring.
  • These lumps or abscess may spread to the waistband, nape of the neck and inner thighs too.

It has three stages

  • Localized or single abscesses along with itching and discomfort.

What are the Causes of Hidradenitis Suppurativa?

The exact cause of Hidradenitis suppurativa is not recognized. But there are some potential reasons which can prone a person to this disease.

  • Smoking and obesity both are associated with this disease as it can make symptoms of the disease worse.
  • There can be considered the role of sex hormone as this disease usually occur at puberty and is less common after menopause.

Diagnosing a rare disease like HS can be challenging. There is no definitive test for it yet. It can only be diagnosed by patient’s medical history, physical examination and laboratory test like swab test of the infected area.

What Complications can be of Hidradenitis Suppurativa?

  • The affected area can get infected.
  • Boils or abscess heals very slowly which leaves scars and marks behind.
  • When the abscess is in armpits or thighs it can cause soreness and restrict the movement of the part.
  • Obstruction of the lymph node is very common in this disease which can cause swelling of the legs, arms or genitals.
  • Due to the restriction of movements and bad odor from the boils can be the point embarrassment for the patient which can cause social isolation.
  • In rare cases patient with this disease can develop squamous cell carcinoma of the affected area.

There is no exact cure or consistent effective treatment for Hidradenitis suppurativa in modern medicine. Management includes antibiotics and anti-inflammatory medicines and surgery is another method.

How does Ayurveda Explain Hidradenitis Suppurativa?

According to Ayurveda, Hidradenitis suppurativa is Kapha and pitta dosha. The faulty digestive fire (Agni), i.e. Pitta impairs metabolism and vitiates the Kapha Dosha. The fat molecules are impaired and start depositing in the muscles and block the channels of the body which causes painful swollen lesions in the groin region, buttocks, thighs, and armpits usually.
