Abstract

Shaken baby syndrome is a brain injury due to abuse. It is a serious condition in which a caregiver or parent shakes the infant out of frustration or anger usually due to baby crying. It is usually seen in babies less than two years of age. The average incidence of this disease takes place between 3 to 8 months old. The abuse is usually by the mother, the father, or the babysitter. Parents feel stress or anxiety because of the social, environmental, financial, or biological conditions that may be much susceptible to violent and impulsive behavior. So today we will briefly read about shaken baby syndrome in this article.

Introduction

Shaken baby syndrome is also called ATH (Abusive head trauma) in which a baby gets deadly head injury in the baby less than two years of age that occurs when an infant or child is violently shaken. That leads to bleeding, swelling and bruising in the brain. It is a type of abuse to the baby. When a child is hardly shaken by the arms, legs, or shoulders, it can cause vision problems, learning disabilities, speech and hearing issues, cerebral palsy, behavior disorder and irreversible disability. It can be deadly in some cases.

Causes and risk factors

Due to anger or frustration, the parents, caregiver, guardian shakes severely to an infant or toddler till the baby does not stop crying. There are some risk factors which triggers the infection which are as follows

Ayurvedic overview

In Ayurveda, shaken baby syndrome symptoms are closely related with vitiated vata dosha. Ayurveda has given prime importance to siras (Brain or head), considering it as one of the three principal vital organs (Trimarmas) of the body where the prana (Vital) resides. Brain is produced by the essence of slesma (Kapha) and rakta (Blood). Touch, organs of perception, pulsation in the body, lightness and all movements are derived from qualities of vata. Prana vata is situated in the whole body. It commands the intellect, mind, brain, sense organs and is responsible for sneezing reflex, swallowing, salivation, production of speech, mental stamina, memory, respiratory movements etc and is responsible for all vital functions.

Vitiated vata symptoms are body tremors, involuntary movement spectrum, confusion, altered higher mental functions, decrease of strength and capacity of sense organs, delusion, delirium, insomnia etc.

Samprapti (Pathogenesis) of vata vyadhi

Because of nidana (Cause), the channels become rikta (Vacant) and vata gets vitiated. This can be compared with the degenerative changes in the nervous system leading to various disorders.

Treatment

Post-Traumatic Stress Disorder

Introduction

Post Traumatic Stress Disorder is a psychiatric disorder in which one witnesses a traumatic event once in a lifetime. It is known as shell shock and battle fatigue syndrome which develops when one experienced terrifying events such as sexual assault, traffic collisions and child abuse. People with PTSD have disturbing thoughts and feelings which are related to experience even though the trauma has ended. There are four types of post-traumatic stress disorder – intrusive memories, avoidance, negative thinking and mood, and various changes in a physical and emotional state. It occurs at any age, in every person of every nationality. Women are more prone than men to getting this disease. PTSD (Post-traumatic stress disorder is a lasting consequence of trauma which causes fear, helplessness, and horror. Those who suffer prolonged trauma and abuse may develop complex post-traumatic stress disorder (C-PTSD).

Pathophysiology

It is divided into two parts

1. Neuroendocrinology

This may be a result of a traumatic injury which causes a reactive adrenaline response which creates various neurological patterns in the brain. During traumatic experience there is a high level of stress hormones which are secreted and suppress hypothalamic activity, causing biochemical changes in the brain and body. It causes low levels of serotonin.

2. Neuroanatomy

Reduced total brain volume and intracranial volume, but with a patient of PTSD have decreased activity. In reports of PTSD, there is a large degree of social anxiety disorder.

Ayurvedic Management

  • Post-traumatic stress disorder is a serious disorder which occurs due to various life happenings such as physical assault, roadside traffic accidents and away from loved ones. This includes various signs and symptoms which include anxiety, hallucinations and headache.
  • In ayurveda post-traumatic stress disorder is correlated with the Aggravation of Vata doshas.
  • It includes Vata doshas which further includes majja dhatu and manovaha srotas, manovaha vikara arises because of fear and grief which leads to vitiation of Vata doshas and the mind is the abode of disease which causes physical ailment. The mind works as a dual organ which has extraordinary sense organs. Manas is said to be present throughout the body except for nails, hair and other structures, hence it is the seat of all sense organs.

Treatment

  • Virechana (purgation therapy)

  • Abhyanga- daily massage which is helpful in Vata imbalance.
  • Shirodhara- it is treated by pouring buttermilk on the forehead, it helps to balance Vata doshas and clear the srotas and helps to eliminate the sensory overload.
  • Use of ghrita such as panchagavya ghrita, kalyanka ghrita which are helpful in post-traumatic stress disorder used as nasal drops.

1. Daivavyapasraya chikitsa (Spiritual therapy)

3. Satvavajaya Chikitsa (psychobehavioral therapy)
