“हनुमुले स्थितो बंधनात स्रंसतयनिलोहन |

विवृतासयत्वमथवा कुर्यात स्तब्धमवेदनम ||

हनुग्रह च संस्तभ्य हनु संवृत वक्त्रताम ||”

(च. चि. २८ /४९)

It states that elevated vata dosha is situated at the temporomandibular joint leading to severe pain at the affected area. It is of 2 types: In one there is: Vivritasyta ‘विवृतास्य्ता’ (mouth remains open), Samvritasyta ‘संवृतास्य्ता’ (in which mouth get closed). Oromandibular dystonia possesses an aggregation of different kinds of symptoms. As per ayurveda it comes under “vata vyadhi” primarily it evolves due to vitiated vata dosha. Vata is considered as one of the most important energy in the body as it is the only movable energy that takes pitta and kaph along.

पित पंगु कफ पंगु पंगवोमलधातवः

वायुना यत्रनियंत्रते तत्र गछन्ति मेघवतः ||”

This is a very beautiful shloka mentioned in our text that tells us about the importance of vata. It states that both pita and kapha along with other “मल” (waste products) dhatu’s(essential substance) of our body are “पंगु” (i.e. unable to move) ,only vata (वायु) take them from one place to other, same as the air in atmosphere take megh (clouds) to different directions.

Causes

Exact cause of the disease is not known but we can consider that due to increase in conduction of neurotransmitters to oromandibular region, the functioning of that area got increased which results in elevation of involuntary movements, Hereditary cause- sometime there may be a family history of disease inheritance, By intake of certain neurotoxic & psychotropic drugs,Trauma, It can also see as symptoms in certain disorders like in tetanus.

According to ayurveda- these kinds of disorders occur due to vitiated vata dosha in the body.

  1. Intake of ruksh (rough), sheet (cold), laghu (low weight) food items.
  2. If one does अध्यशन (overeating, even if previously eaten food is not digested) प्रमिताशन (eat in less quantity).
  3. Over indulge in strenuous exercises.
  4. When one chew hard substances.
  5. One controls his/her natural urges for a longer time.
  6. If a person sleeps on a hard surface or sits on an uncomfortable chair/bench.
    When a person has had an underlying disease for a long time.

  • Spontaneous opening or closure of mouth
  • Pain in facial region
  • Difficulty in speaking
  • Uncontrolled eye movements.
  • Problem with grinding food and swallowing.
  • Diminished sensation in jaw.
  • Struggling with teeth grinding and chewing.

  • By clinical evaluation of signs and symptoms.

  • Intramuscular electromyography( EMG)

Treatment of Oromandibular Dystonia

According to Ayurveda

  1. Firstly do निदान परिवर्जन( avoidance of the cause)
  2. Local massaging (अभ्यंग) with any herbal oil by keeping a slow pace would be appreciated. e.g., with महामाष तैल
  3. Oil pulling- 2 ancient methods used in this regard are as follows
  4. गण्डूष (mouth is completely filled with oil, keep it in mouth until you start having sweating on your forehead) e.g. with महानारायण तैल
  5. कवल (gargles is done with oil)
  6. Local (स्वेदन) – use naadi sved. This medicine is sprayed on the patient’s body to make it free of toxins.
  7. Using शिरोधारा- pouring medicated herbal oil over the head of a patient by a specially made shirodhara yantra.
  8. BASTI KARMA (बस्ति कर्म) – BASTI is considered as “अर्ध चिकित्सा” means only basti can remove all the ailment of our body.

Maintenance of Oral Cavity or Maintenance of Oral Hygiene

1. Brushing of Teeth (Dant Dhawan)1

One should brush their teeth two times a day i.e. in the morning or at night before going to sleep. Brushing should be done with twigs of Arka (Calotropis procera), khadira (Senegalia catechu), karanja (Millettia pinnata) and arjuna (Terminalia arjuna). The breadth of the twig is about our little finger and length is about 12 angula (12 fingers). The taste selection for the brushing of teeth is tikta (bitter), katu (pungent), kashaya (astringent). In absence of twigs we should use Triphala powder, trikatu and trijataka. It clears all the toxins from the mouth because the mouth is the main source of infection.

2. Tongue cleaning (jihva nirlekhan)3

After brushing our teeth, we should take care of our tongue too. Tongue cleaning should be done with a soft and smooth instrument which is made of svarna (gold), rajat (silver) and loha (iron). The length must be 10 angula.

3. Gandush (gundusha)4

Gandusha means mouth is completely filled with dravya and kept for about 5 minutes without any movement inside mouth.
