Abstract

CVI is a condition in which there alteration in the physiological function of the veins. In Ayurveda it is considered as vatarakta and variants of ulcers. It is medical condition in which veins are unable to properly transport blood from the legs back to heart.

Chronic Venous Insufficiency

Introduction

CVI means chronic venous insufficiency. It is a common condition in which venous valves in the leg do not function effectively, making it difficult for blood to return to the heart from the legs.

Veins are basically the blood vessels which carry deoxygenated blood towards the heart. Though Ayurveda does not describe CVI as a unique disease but according to studies it can be considered as vatarakta and variant of ulcers.

Symptoms of CVI

If any person develops medical condition like chronic venous insufficiency then the symptoms can grow from minor to major if left untreated. So it’s very important to treat chronic venous insufficiency at early stage. Symptoms of chronic venous insufficiency are described below:

  • Heaviness and swelling, especially in the lower legs and ankles
  • Skin looks like leather

If chronic venous insufficiency is not treated, the pressure and swelling increases until the tiniest blood vessels in the legs (capillaries )burst and when this condition arises the overlying skin become reddish Brown and becomes very sensitive to been broken if scratched. Now, these blood capillaries causes inflammation in local tissues and internal tissues and damages them. At worst stage this leads to ulcers, open cellulitis.

Causes of CVI

CVI can be caused by long-term blood pressure that is higher than normal inside the leg veins. The following factors are more likely to cause venous insufficiency:

  1. Having a family history of the condition
  2. Having blood clots
  3. Having obesity
  4. Having varicose veins
  5. Sitting or standing for long periods of time
  6. Being pregnant
  7. Not doing enough exercises
  8. Persons over 50 years old have a higher risk of developing chronic venous insufficiency

Complications of CVI

Poor circulation in the lower extremities may cause chronic venous insufficiency which left untreated can evolve severe complications which are as follows:

  1. Venous Ulcers: This is one of the complication of CVI which is the dangerous condition. These ulcers may be painful and slow to heal if they are not handled with proper care, these ulcers can eventually become infected.
  2. Blood Clots: Blood clots are formed due to reduction in blood flow in the legs. Chronic venous insufficiency is sometimes linked to deep vein thrombosis. DVT is formed when a blood clot forms in one of the deep vein of the leg.
  3. Bleeding: Circulation ability may affect the procedure of injury healing. If the medical condition like chronic venous insufficiency persists within you and you got injured then you may bleed heavily. Uncontrolled blood loss can sometimes be life-threatening.

Diagnosis of CVI

To diagnose chronic venous insufficiency, doctor will perform a complete medical history and physical examination. During the physical examination, the doctor will carefully examine your legs. And imaging test called a duplex ultrasound may also be done. This looks at blood flow and the structure of leg veins. It checks the speed and direction of blood flow in the blood vessels.

But history and examination by doctor for characteristics signs and symptoms are sufficient in many cases in ruling out systemic cause of venous hypertension such as hypervolemia and heart failure.

Common variable immune deficiency (CVID) is a disorder in which the body’s immune system is unable to defend against bacteria and viruses, especially in adults. This disorder is characterized by low levels of serum immunoglobulin (Ig) and lack of B lymphocytes ( cells responsible for producing antibodies)  due to which person becomes more susceptible to recurrent infections affecting the ears, sinuses, and respiratory tract (Lungs). Pneumonia is a common symptom of CVID.

Both humoral and cell-mediated lymphocytic responses are affected due to defective antibody formation in a patient of CVID. There is a problem in the T-cell ability to help B-cells and B-cells respond to T-cell help.

CAUSES OF CVID

Although Common Variable Immune Deficiency (CVID) is mainly considered as a genetic disorder and the genetic mutations that lead to CVID have been identified, the exact cause of the disease is still unknown in most of the cases due to the heterogeneity of the disease.

Mutation in genes occurs in CVID which affects B Cells. These are specialized white blood cells that protect the human body from infections. Mutations in the genes lead to dysfunctional B cells which in turn are not able to make a sufficient amount of antibodies.

In some patients, there is a deficiency of the antibodies called IgG (immunoglobulin G), IgA (immunoglobulin A), and IgM (immunoglobulin M). The person is unable to fight against infections due to a shortage of these three antibodies (IgG, IgA, and IgM).

SYMPTOMS OF COMMON VARIABLE IMMUNE DEFICIENCY (CVID)

People mostly suffer from Pneumonia in CVID which in the future leads to chronic lung diseases (bronchiectasis). Chronic infection of the lungs (recurrent pneumonia) can lead to lung damage and many other complications.

CVID is considered the most common form of PIDD (primary immunodeficiency diseases) causing a low number of platelets or white cells, anemia, arthritis, and other endocrine disorders.

CVID can be associated with gastrointestinal problems like chronic diarrhea, weight loss, nausea, vomiting, and pain in the abdomen. People with CVID may develop granulomas in the lungs, lymph nodes liver, skin, or other organs. In some forms of CVID, patients are also at increased risk of lymphoid and gastrointestinal cancers

DIAGNOSIS

Diagnosis of CVID can be made through screening tests that measure immunoglobulin levels (IgG, IgA, and IgM) or the number of B cells in the blood. Decreased serum levels of IgG and IgA are the main criteria, there are low levels of IgM and dysfunction of T-lymphocyte.

TREATMENT OF CVID

Immune Globulin Replacement Therapy is the main line of treatment in which lifelong Ig (injection of human antibodies) is replenished that the patient is lacking. This therapy is used to prevent infections and treat bacterial infections involved with this disease.

Injections of Ig are given by both routes intravenous (3-12 % Ivy) and subcutaneous (10-20 % subcutaneous Ig (SGIg). Along with Ig replacement therapy (IRT) immunosuppressant, corticosteroids, and high doses of antibiotics are given to reduce the rate of acute and chronic infections.

There are many adverse effects of immunoglobulin therapy like

  • Fever, Headache, nausea, chills
  • Fatigue
  • Muscle aches and pain, or joint pain
  • Thrombotic events (rare)
  • Aseptic meningitis (rare)
  • Anaphylactic shock (very rare)

In modern science, the only treatment for CVID is IRT (Ig replacement therapy), which is a lifelong treatment and sometimes not very effective. The prolonged use of NSAID and CORTICOSTEROIDS can damage the liver cells and can depress the immune function.
