Dementia

Introduction

Dementia is a clinical diagnosis and a term that can be used to explain multiple symptoms affecting the thought process or memory. Dementia is not a disease, it can be caused by multiple problems affecting our mental aspect. In simple terms, dementia is explained as the loss of memory. Dementia can be a sign of problems associated with impaired brain function. Therefore we can say dementia is the progressive memory loss associated with neurological disorders. Causes

  1.  Disruption or disturbance in nerve connection may lead to dementia. It may be caused due to loss of nerve cells. Symptoms may vary from person to person as it depends on the damaged part of the brain.
  2.  Protein or protein deposits may cause poor nerve connection in the brain and may lead to distorted brain systems.
  3.  Side effects from the disease may cause altered thought processes or memory loss
  4.  Stress and overthinking may lead to such problems.
  5.  Alzheimer’s disease – The common cause of dementia is Alzheimer’s disease. It is prevalent in old age people. It is sometimes hereditary. People with age more than 65 are more prone. Brain tests or scanning of the brain show the plaques. Plaques are associated with beta-amyloid. Word-finding problems are associated here. Although the exact cause of Alzheimer’s is unknown, brain studies reveal the plaques. Plaques can be due to tau protein mass or a group of beta-amyloid proteins. Alzheimer’s progresses slowly and symptoms may take time. Patients of Alzheimer’s face problems like dementia, feeling lost and being unable to retain things.
  6.  Vascular dementia – Vascular dementia occurs when there is low blood flow or obstructed blood flow to the brain, it leads to brain damage. Another name for Vascular dementia is multi-infarct dementia. Multiple small strokes may lead to a vascular type of dementia. People with high blood pressure or face stroke or heart attack are more prone.
  7.  Lewy body dementia – a common form of irreversible dementia, its primary symptoms include visual hallucinations and “Parkinsonism” where the patient exhibits features of Parkinson’s disease like tremors and rigid muscles. People diagnosed with Lewy body dementia experience a sleep behavior disorder that involves acting out dreams.
  8.  Frontotemporal dementia – With this condition, the nerve cells in the frontal and temporal lobes of the brain degenerate, which results in inappropriate behavior, language problems, and difficulty with thinking, concentration, and movement problems.

Complications

  1.   Restlessness and agitation. People diagnosed with AD commonly have periods of agitation and anxiousness.
  2.   Bladder and bowel problems. Bladder and bowel problems are other complications of AD.

Management

Manovaha srotas dushti

Manovaha srotas can be designated as the channels of consciousness. The channels of consciousness flow via the mind. Manovaha srotas are important for basic actions and functions like thinking, behavior, remembering, and reasoning. The impaired status of poor functioning of manovaha srotas leads to various symptoms. Srotas are important in Ayurveda because they make up a system of channels that allow the body to function optimally. Manovaha srotas are concerned with Sattva, Rajas, and Tamas gunas. Partial Empty Sella Syndrome

INTRODUCTION

Empty sella syndrome is one of the rarest disorders that includes the part of the skull named as sella turcica. The sella turcica is located at the base of the skull  in the sphenoid bone which holds on the pituitary gland. In case of partial empty sella syndrome the part of the skull sella turcica is partially filled by CSF (cerebral fluid). The pituitary gland gets smaller in size in case of partial empty sella syndrome. When there is some underlying condition the partial empty sella syndrome is known as the secondary empty sella syndrome and when there is no associated cause then it is known as primary empty sella syndrome. Further we will discuss the cause and symptoms of empty sella syndrome.

CAUSES

As we read earlier it is of two types primary and the secondary one so the causes are different for both the types. The causes for primary empty sella syndrome are not clear yet but it can be related to birth defect that is in the  diaphragma sellae which is a membrane covering the sella turcica. Some of the patients are born with a small tear in the covering of sella turcica that leads to CSF leakage in the sella turcica. And it is not clear that this is the exact cause of this syndrome but considered as one of the risk factors. Now let’s see the causes of secondary empty sella syndrome, these may includes head trauma, infection, pituitary tumours, radiation therapy, intracranial hypertension, sheehan syndrome, neurosarcoidosis- these all are the conditions of the brain and pituitary that may lead to empty sella syndrome.

SYMPTOMS

Empty sella syndrome although doesn’t show any symptoms but in case of secondary empty sella syndrome a person may experience symptoms associated with the particular condition. Most of the patients with this syndrome suffered from chronic headaches but nobody is sure if it is due to the empty sella syndrome or due to high blood pressure. Other symptoms associated with the pituitary gland as its shrinkage occur in this syndrome. Rarely the empty sella syndrome associated with the building up of pressure in sella tunica that leads to the following symptoms:

  • Leakage of spinal fluid from the nose
  • Problematic vision
  • Swelling of optic nerve in the eye
  • Low production of pituitary hormones
  • Increase in pressure of spinal fluid
  • Impotence in Men
  • Hampered menstruation in females

DIAGNOSIS

Empty sella syndrome is not easy to diagnose as it does not produce any specific symptoms. If there is a suspicion of the disorder in some cases then the case will be further diagnosed with physical examination and after that one review is taken on the medical history of the patient. For clear diagnosis CT Scan and MRI is to be done. This examination also helps in diagnosing if the empty sella syndrome is partial or total. One other thing is to be added in a clear diagnosis that is the examination of pituitary hormones such as Adrenocorticotropic hormone (ACTH), Thyroid-stimulating hormone (TSH), Luteinising hormone (LH), Follicle-stimulating hormone (FSH), Prolactin (PRL) and some other. So these are some steps of diagnosing the empty sella syndrome to be followed for clear diagnosis.

As per ayurveda there is no direct correlation of Empty Sella Syndrome, there is the involvement of all the tridosha in thi syndrome. As the pathology of the disease is like there is the accumulation of CSF in sella turcica and then the size of the pituitary gets shrinked, at last there is depletion of pituitary hormones. So the flow of CSF into sella turcica is due to vitiation of vata dosha, the accumulation is due to increase in kapha dosha and the low secretion of pituitary hormones are due to vitiation of pitta dosha.
