Abstract

Costochondritis is a condition in which there is inflammation of cartilage that joins the sternum. The pain due to this inflammation may be like that of heart conditions. Costochondritis

Introduction

Costochondritis is a condition of inflammation of cartilage that connects the rib to the sternum. It is also known as chest pain, costosternal-chondrodynia or costosternal syndrome. Usually it has no well explained cause and it remain idiopathic. In most of the times pain is usually on left side of the ribs. It might last few weeks to several months. The pain occurs in the costochondral junction (the junction between cartilage and sternum). The patient must complain of chest pain and there is no chance that one should ignore any type of chest pain as sometimes it may be dangerous. In allopathic system doctors focuses on pain relief only. As per ayurveda the disease is correlated to asthigata vikara (diseases of bones) as it is the disease of cartilage known as upasthi and the disease is cure by the treatment given in asthigata vikara.

Symptoms of Costochondritis

The main symptom of costochondritis is chest pain along with that there are some other additional symptoms. Let’s take a look on those symptoms:

  • Tenderness on pressing rib joints.
  • In case of infection there is swelling, redness or pus.
  • The pain might be worse when lying down.
  • The pain gets aggravated when the patient applies pressure on the chest.

Causes of Costochondritis

  • Joint infection like some kind of viral, bacterial or fungal infection
  • Tumors: Cancerous or noncancerous tumors may cause costochondritis
  • Most commonly occurs in women

Diagnosis of Costochondritis

The patient of costochondritis is diagnosed mainly physically. Doctor will diagnose physically while touching along the area that is inflamed and tender. He will move your arms and ribs in certain ways to diagnose the type of pain. As the pain occurs in costochondritis mimics other issues such as heart disease, gastrointestinal condition or lung disease. So the doctor will ask you for a certain test for distinguishing costochondritis from other health issues. The diagnostic techniques used are- X-ray, CT-scan, MRI and even electrocardiograph.

Ayurvedic view of Costochondritis

As per ayurveda costochondritis is correlated to asthigata vikara. These vikaras occur with dushti of asthivaha srotas as explained earlier that costochondritis is disease of upasthi (cartilage). The Causes of asthivaha srotas dushti as per ayurveda are- excessive physical activity, excessive anger, trauma, pressure, bone friction, excessive diet of vata dravya. As all the causes are responsible for vitiation of vata dosha so the treatment option is to balance the vata dosha such as panchkarma, tikta ksheer basti and tikta sarpi (ghee) paan (intake).

  • Costochondritis Stretches: These stretches are very helpful in managing sternum pain. Here are some exercises like wall side- in this exercise you have to stand with your back against the wall and raise your arms up with elbows raised up to the shoulder height and then you have to keep your arms against the wall then raise the arms overhead. Hold your arms and then come to the starting position. Repeat this for 10-12 times. Let’s discuss another exercise that is scapula squeeze- in this exercise you have to stand straight and breathe comfortably. Squeeze the both shoulders together and hold them as long as you can and then release. Repeat this exercise for 5-10 times.
  • Use of heat or ice packs: pain and inflammation of costochondritis is relieved with this heat or ice packs. You have to use a heat or cold pack by putting hot or cold sauce in a towel to avoid skin damage. 
  • Rest: Avoid doing work to keep your rib cage in resting position to heal yourselves completely.

Strong bones are very important for living a healthy life. Low bone density is the leading cause that results in the state of osteoporosis i.e. formation of holes in the bones. It will result in various pathological fractures such as when the fracture of a bone occurs due to pathological changes in the bone structure. A person with low bone density becomes prone to bone fractures; he/ she will get his/ her bone fractured even by a slight pressure by a very low amount of force e.g. falling while walking or even without any visible reason. INTRODUCTION

The structure of the body is provided by the bones. There are a total 206 bones in the body. And each and every bone provides strength and structure to various parts of the body. To measure the health status of the bones, a bone mineral density test is performed. The bone mineral density test is used to measure the levels of calcium and other types of minerals that are present in an area of bone. The normal range of T- score is suggested to be -1.0 or more. The reports that result in lower values suggest a lower bone density. A lower bone density may worsen and result in osteoporosis i.e. condition of appearance of pores in the bone.

Low Bone Density

CAUSES

Normally, the formation of bones and reabsorption of bones are simultaneously ongoing processes. The bones start getting thinner as the age advances. This phenomenon occurs because the rate of formation of bones becomes slower than the rate of reabsorption of bones. The different causes that are estimated to cause low bone density are as follows- various types of eating disorders, chemotherapy, and extreme exposure to radiation. Low bone density may be a consequence of a disease. Women are more prone to it because of the continuous hormonal changes occurring in the body. Specially, the women around the age of menopause become more susceptible to thinner bones.

SYMPTOMS

There are no specific symptoms related to the condition. The person with low bone density will not experience pain or any other symptom because of reduced bone density. But he/ she will be at a higher risk of fractures of bone. The person breaks his/ her bone even when the force or the pressure suffered is very less. For example, he/ she might get his/ her bone fractured even if he/ she falls while walking.  The bone gets thinner as the bone density levels fall and hence the chances of fracture are increased.

DIAGNOSIS

To diagnose the bone density of an individual’s bones, a bone mineral density test is performed. The reports will consist of two parameters. The bone mineral density test is used to measure the levels of calcium and other types of minerals that are present in an area of bone. One of them is T-score and the other one is Z – score. The normal range of T- score is suggested to be -1.0 or more. Any value, lower than this, will suggest low bone density and bones thinner than normal.

TREATMENT

The management of the condition of low bone density will include various measures such as administration of Vitamin-D, Calcium and various lifestyle changes. The lifestyle changes will include adopting healthy food habits and other health improving habits. The person is suggested to increase the intake of healthy foods and inhibit the junk foods totally. He/ she is given a calcium complex. The patient is advised to avoid intake of alcohol and smoking. They are suggested to exercise on a regular basis. Exposure to sunlight is a good source for vitamin-D.

AYURVEDIC ASPECT

Ayurveda always has a very unique perspective on all the processes of the body. The concept of bones and bone density is discussed in a very interesting manner in the classical texts of Ayurveda. The bones can be correlated to Asthi in the Ayurvedic literature. The asthi is mentioned as the fifth dhatu that are rasa, rakta, masa,meda, asthi, majja, shukra. These are described as the elements that dharana (retention) the sharira. They are responsible for an appropriate healthy state of the body. The asthi is said to be a very hard component that contains vata (which can be correlated to space and hence pores) and majja dhatu (a type of liquid that can be correlated to bone marrow) inside it. There is a very unique concept of Ashraye- ashrayi bhava in the description of dhatus.

It explains that each of the seven dhatu is a home to another component of the body. And as the level of the component rises, the levels of that dhatu also rise. But there is an exception in the case of asthi. The asthi, as discussed above is supposed to be a sthana (home) for vata dosha, contrary to the principle, the levels of asthi and the vata are indirectly proportional. That means when the levels of vata rise, the levels of asthi decrease. So, in low bone density conditions, it can be estimated that the levels of the vata dosha are on a higher side which leads to decreased levels of the asthi component. This interprets that to raise the asthi component (i.e. can be correlated with bone) one should put efforts to decrease the levels of the vata dosha. There are various herbal drugs that are beneficial in decreasing the levels of vata dosha in the body such as Ashwagandha, agaru etc. The compounds like praval pishti, etc are known to improve the calcium levels of the body. Hence, help in improving the bone health of the individual. He/ she are advised to practice a healthy lifestyle and healthy food habits.
