Ayurvedic Treatment for Somatoform Disorders – Somatic Symptom Disorders (SSD)

ABSTRACT

Somatoform disorders are commonly known as Somatic Symptom Disorder. It is considered in psychiatric disorders and involves physical and mental illness. Somatoform disorders include different types of disorders such as somatization disorder, undifferentiated somatoform disorder, conversion disorder, pain disorder, hypochondriasis, and body dysmorphic disorder. The symptoms are unexplained and found in early patients. If the patient has a somatic disorder and experiences physical and mental distress the treatment can cure the symptoms and improve the quality of life of the patient.

INTRODUCTION

In Ayurvedic terms, the Vata Kapha dosha and tamas guna are responsible for the somatoform disorder. The symptoms involve one or more organs or body systems such as Pain, Neurogenic problems, and Gastrointestinal complaints. Somatoform disorders occur in 0.2 – 2 % of women and less than 0.2 % of men.

CAUSES

The Cause of somatoform disorder is unknown but some risk factors are there which cause somatoform disorder. The risk factors are follows as

  1. Genetic and biological factor
  2. Emotional stress
  3. Childhood trauma
  4. Childhood illness
  5. Mental health condition
  6. Anxiety
  7. Depression

SYMPTOMS

Somatoform disorder with the following symptoms

  1. Pain in any part of the body or the whole body
  2. Shortness of breath
  3. Anxiety
  4. Abdominal pain
  5. Altered bowel habits
  6. Digestive problems
  7. Abnormal movements such as tremors and seizures
  8. Blindness
  9. Hearing loss
  10. Loss of sensation in the body

TYPES

  1. Somatic symptom disorder (SSD):- also known as somatization disorder. The disorder occurs between 18-30 years of life. More women than men are affected. The cause is not known but affects the person’s day to day life activities. The diagnosis is difficult as there is no physical cause for the symptoms. Some Somatic symptoms are diagnosed when the person has excessive thoughts, behaviour, and feelings relating to physical symptoms without a medical condition such as pain, weakness, and shortness of breath.
  2. Conversion disorder:- Also known as functional neurological symptom disorder. it occurs between the ages of 18 to 30 years. It is a condition where a person has symptoms that suggest serious neurological diseases such as brain or nerve diseases. For example total loss of vision (severe sight impairment), deafness, weakness, numbness, or paralysis of arms and legs. Symptoms appear for a few weeks but in some cases, it appears for a longer time.
  3.  Pain disorder:- It is a condition where a patient has persistent pain but not attributed to a physical disorder. Due to pain patients experience stress and discomfort. It is due to psychological factors. The severity and duration of psychological factors will affect the severity and duration of pain.
  4.  Hypochondriasis:-  Also known as anxiety illness disorder.  People think a normal symptom is a sign of a serious disease. For example -they believe that a common headache is a sign of a brain tumor, and a normal skin rash is a sign of skin cancer. Normal body sensations such as tummy rumbling may be thought of as a symptom of serious illness. The disorder is similar to somatization disorder but in this condition, people may accept that the symptoms are minor but fear they are caused by serious disease.
  5.  Body dysmorphic disorder:-  It is a condition where a person spends a lot of time worried and concerned about their appearance. A person spends most of the time thinking of minor or imagined defects in the body. The patient frequently examines their appearance in a mirror, consistently compares their appearance with others, and avoids social photos and communication.

DIAGNOSIS

  1. History of many physical complaints – Any drug or alcohol abuse, stress.
  2.  Physical examination – Pain, sleepiness, and fatigue that cause distress and have a negative impact on daily functioning.
  3. Psychological examination – To talk about symptoms, fears, concerns, stressful situations, relationship problems, and family history.

TREATMENT

  1. Psychotherapy:- It includes cognitive behavioural therapy (CBT) or Trauma therapy
  2. Medication:- Antidepressant medication
  3. Relaxation therapy- It includes deep breathing techniques, meditation, massage, and music therapy.

AYURVEDA ASPECT FOR SOMATOFORM DISORDERS

In Ayurvedic aspects, Somatoform disorders come under Manovikaras. Due to an increase in vata kapha dosha (sharirik dosha) and rajas guna (mansik dosha) mental illness will occur.As Acharya Charak explained in Dashavidha pariksha and tells about the bala (strength ) of a person in which  Avara sattva people are commonly affected with somatoform disorder. Some symptoms are seen such as Anidra ( insomnia) due to vata dosha, Murcha due to  Pitta dosha, tandra, and nidra (sleep) due to Kapha dosha.

Causes of somatoform disorder according to Ayurveda

  1. Mithya aahar ( improper diet )
  2. Mithya vihara ( improper activities)
  3. Ratri Jagran ( inability to sleep at night )
  4. Manashika hetu ( psychological stress )

Treatment

In Ayurveda, Manovikaras are treated as follows

  1. Daivavyapasraya chikitsa ( A Spritiual therapy ):-  It includes Mantra, Mani , Aushadhi, Bali, Homa,  Upahara, Upavasa, Niyama
  2. Yuktivyapasraya chikitsa ( A Logical therapy):- Administration of medicine such as nasya, Shirolepa, and as well as diet.
  3. Satwavajaya chikitsa ( A Psychotherapy ):- It is an Adravyabhoota chikitsa.

Other treatment strategies

  1.  Medhya Dravayas:- A group of brain tonics that help improve memory and keep the mind in a state of balance. For example- Mandukparni, Yasthimadhu, Guduchi, Shankhapusphi.
  2. Chittodvega chikitsa:- Refers to a set of anxiety disorder
  3. Unmada(Insanity) /Apasmara(Epilepsy)  chikitsa
  4. Atatvabhinivesha chikitsa:- It means lack of judgment and loss of discrimination between which is good or which is bad.

Product Description

1. Boswellia curcumin capsules

Boswellia curcumin is a useful ayurvedic capsule that contains curcumin (Curcuma longa) and shallaki ( Boswellia serrata). 4. Kalyanaka ghrit

Kalyanka ghrit contains Badi elaichi ( Ammonium subulatum), Hariaki (Terminalia chebula), Vishala (Rakta indravaruni), Devadaru (Cedrus deodara), Bhibhitaki (Terminalia bellirica),  Elavaluka (Prunus avium), Amalaki (Emblica officinalis) and Ghrita ( cow ghee)  and many other herbs. It balances vata and pitta dosha. It boosts memory and mental health and relieves stress so it is useful in somatoform disorder. It is also useful in convulsions, tremors, psychosis, and obsessive-compulsive disorder.

Dosage:– 1 tsp twice with milk (warm)

5. Saraswatarisht

Saraswatarisht is a great medicine that contains Vidarikand (Pueraria tuberosa), Harad (Terminalia chebula), Brahmi (Bacopa monnieri), Shatavari (Asparagus racemosus), Vidarikand (Pueraria tuberosa), and many other herbs. It is helpful in acute anxiety, partial loss of memory, fatigue, and depression. As the vitiated vata dosha is responsible for the behavior of the patient it pacifies the vata dosha and helps in relieving symptoms.

Dosage:– 2 tsp twice daily

Conclusion

Symptoms of the somatoform disorder cannot be ignored because it affects the physical and mental health of the patient and decreases the quality of life of the patient. The exact cause is unknown but some risk factors are there that cause somatoform disorder. In Ayurveda, there is no direct correlation of the somatoform disorder, but as Acharya Charak mentioned Avara satva patients are commonly affected by this disorder. In the ayurveda aspect Satwavajaya chikitsa, Daivavyapasraya chikitsa, and Medhya Dravayas can be effective and in modern cognitive behavioral therapy, psychotherapy can be given.

Ureaplasma Vaginitis – Causes, Symptoms, Prevention & Ayurvedic Treatment

ABSTRACT

Ureaplasma are bacteria groups which reside in the urogenital and respiratory tract of humans. These microorganisms are freely found in nature and in the human body; helps in fighting infection, digestion and mainly keeps reproductive health in check. Normally these microorganisms are harmless but when their quantity increases to more than normal then they form colonies & inflame the surrounding cells/tissue thus leading to infection. This infection may lead to bacterial vaginosis termed as “ureaplasma vaginitis”. Let’s discuss it in detail!

INTRODUCTION

Infection due to Ureaplasma gets transmitted from one person to another mainly via sexual activity. It enters the body via the urethra or vagina and can also pass to the child from the mother during childbirth. As we know that these bacteria are already present in our body normally, we can say that a weak immune system may result in overgrowth of these microorganisms. Ureaplasma vaginitis is a STI/STD (sexually transmitted disease) as it spreads via sexual activity. Ureaplasma belongs to the class of mycoplasma which are the smallest organisms of their type known to man. These classes of bacteria do not have any outer cell wall hence are resistant to various common antibiotics and also to penicillin. Thus this condition of ureaplasma is very tricky to cure. One must firstly try to avoid having such communicable infection by using proper preventive measures and if the infection is caught then must get treated at the earliest.

Ureaplasma Vaginitis

CAUSES

Mainly ureaplasma vaginitis spreads to one another via sexual contact, via urethra and vagina. It can also pass to the baby from the mother during delivery. People with an immune system, or patients who take immunosuppressants are more susceptible to ureaplasma infection.

SYMPTOMS

Most patients  do not experience any symptoms and may act as carriers. Symptoms are as follows-

  1. Pain during micturition
  2. Burning sensation
  3. Vaginal or urethral discharge
  4. Bad odour
  5. Inflammation

COMPLICATIONS

  1. Premature labour (not the ureaplasma itself causes premature delivery but the symptoms like infection of cervix, vagina, inflammation may lead to preterm childbirth)
  2. Increased risk of kidney stones
  3. Respiratory problems in newborns
  4. Premature rupture of foetal membrane
  5. Chorioamnionitis
  6. Funisitis

DIAGNOSIS

  1. Urine routine- analysation of urine for presence of any kind of bacterial infection in the urine.
  2. Swab test- a sample from vagina, anus, throat, cervix is taken using a swab for testing for any infection.
  3. Endometrial swab
  4. Endometrial biopsy- A small tissue piece is taken out of endometrium for cell culture to rule out infection

Prevention

  1. Use condoms- One must use condoms or femedoms during intercourse are the first line of defence against various infections such as H.I.V, chlamydia, gonorrhoea, syphilis, ureaplasma vaginitis etc.
  2. Limit your number of sex partners- do not have multiple sexual partners as it increases the chances of ureaplasma infection.
  3.  Get regular screenings- Person who is more sexually active should get tested for STDs on a regular basis.
  4. Avoid douching- Douching is a vaginal cleansing procedure which must be avoided because it alters the normal flora of vagina which increases the chances of infection.

AYURVEDIC OVERVIEW OF UREAPLASMA VAGINITIS

Vaginitis is the commonest  gynaecological problem reported by women. In Ayurveda diseases of the female reproductive system are  mainly explained under “yoni vyapad”. Vaginal candidiasis, trichomoniasis and bacterial vaginosis (ureaplasma vaginitis) are some common types of vaginal infections. Main cardinal symptoms of Yoni vyapad are:-

  1. Yoni srava (vaginal discharge)
  2. Yoni kaṇḍu (itching)
  3. Daurgandhya (bad odour)

In Ayurvedic classics vitiation of vata dosha (kinetic energy) is said to be the main cause of this condition. Even though ancient scripture directs towards vata vitiation, there is an equal role of other two doshas – pitta and kapha (thermal and potential energy). Vitiation of vata dosha causes symptoms like pain , inflammation etc whereas burning sensation, bad odour, suppuration, fever like symptoms are due to the pitta imbalance. Kapha vitiation causes white discharge, mucoid secretions, itching and feeling of cold etc. Due to this vitiation of doshas there is imbalance in the flora of vagina i.e the acidic nature of vagina changes to basic hence causing an increased number of ureaplasma bacteria causing the infection. Also STDs and other communicable diseases are kept under “AGANTUJA VYADHI” in ayurveda.

3. Chandraprabha Vati

Apart from this chandraprabha vati is also given in abdominal colic, lower back pain  and many gynaecological problems. Contents in this formulation helps in pacifying all 3 doshas- vata, pitta, kapha (kinetic, thermal, potential). Ingredients- kapoor (Cinnamomum camphora), Haridra (curcuma longa), Shilajit (Asphaltum punjabinum), Guggul (Comiphora mukul) etc. This medication has a very soothing effect and relieves the burning sensation. Guggul pacifies vata dosha relieving inflammation and pain etc. Shilajit being a good immunomodulator is also good in infertility, reduces stress and anxiety.

4. Lodhrasav

Lodhrasav also called as rodhrasava, lodhrasav is a classic ayurvedic liquid formulation. It is used in heavy menstruation or in DUB (dysfunctional uterine bleeding). As the name suggests it contains lodhra (Symplocos racemosa) and due to its properties like astringent in nature, hemostatic & anti inflammatory, and pacifies kapha and pitta dosha. It is also used in heavy menstruation or in DUB (dysfunctional uterine bleeding) as it relieves pain and heavy blood flow. Lodhra pacifies pitta and kapha dosha to a great extent hence is used in this condition.

Dosage– 2 tsp twice daily

5. Sphatika bhasma

it is a powdered form of classical ayurvedic medication also called kankshi, tubra, shubhra, phataki with chemical formula – K2SO4.Al2(SO4)3.24H2O . It is used in various ailments like haematemesis, menorrhagia, dry cough, leucoderma, visarpa etc. Contains Sphatika (Alum) which pacifies vata and pitta. Apart from a potent detoxifying agent Sphatika also stops bleeding and is a good wound healer. It is a potent gynaecological medicine as it removes toxins out of the body, reduces inflammation and is antiseptic in nature.

6. Chandanadi Vati

Chandanadi Vati is a herbal tablet formulation which is made by planet ayurveda using ingredients such as Safed Chandan (Sandalwood), Gokshura (Tribulus Terrestris), Choti Elaichi (Elettaria cardamomum) and many more. It is helpful in balancing Pitta and Kapha doshas and maintains healthy metabolism of the body. Promotes healthy digestion and alleviates respiratory problems.

Dosage- 2 tablets twice daily after meals with plain water.

Usage – This sphatika bhasma is used as genital wash in this condition. 1-2 pinch of this powder to 5 litres of water and have a sitz bath in the tub for 25-30 minutes.

CONCLUSION

Ureaplasma is the most common cause of bacterial vaginosis which is curable. It starts as a normal uti infection and gradually increases displaying all the above mentioned symptoms. To rule out any chances of infection regular investigations must be done if one is indulged in regular sex with multiple partners.  And if diagnosed with ureaplasma infection; treatment must be started at the earliest to avoid major complications. This condition is difficult to treat but herbal remedies developed by Planet Ayurveda are very effective in treating ureaplasma vaginitis.
